Quick ones...

When you reincarnate someone as a bugbear or troglodyte, do they get the racial hit-dice, or just the ability mods and appearance...? What about special abilities, attacks, etc...? Do you assign them a level adjustment at all...? We have a druid that reincarnates all the dead people... It's fun, but gets hard to keep track of...

Do animal companions lose a level/HD when brought back to life...? Familiars don't but it says nothing about Animal Companions...

How long does it take for a character to fall 100ft...? If someone falls, is there a certain cut-off distance at which it becomes impossible to fly and catch them...?

Even the main D&D FAQ treats this particular question as both a physics and rules question.

Many of your questions about reincarnate are answered in the spell's description, except for racial Hit Dice. The spell does say your hit points do not change, and that you lose a class level or Hit Dice, but that's all it says. I wish I could help with this one.

To my knowledge, animal companions cannot regain Hit Dice lost from raise dead or other spell.

I would rule that an animel companion brought back would regain the lost HD when the character raised his or her level.

As much an NPC as your companion may be, it is a class featur. I wouldn't wanna gimp a class feature just because it died one time. Especially if it doesn't earn its own HD, that's your job.

I'd house rule something to that effect. Though I'd probably allow the druid or ranger to "buy back" their companion's lost Hit Dice the next time they level up, plus any bonus Hit Dice they might normally receive.

the subject’s soul is free and willing to return

This is the main aspect - not many animals or people would return to life from the after-life...
